CONSENT AND RELEASE FORM FOR CONTESTANTS

I am aware of the risk associated with Covid-19, being around other contestants, the physical injury, including death that could possibly occur from my oarticipation in this event. I voluntarily assume responsibility for any loss of property damage, or personal injury that could occur from being a contestant in this competition. I hereby release, waive, and discharge any liabilities to not sue NCS or any of the members of its committee, volunteers, security, staff, representatives, judges, officers, or anyone affiliated with NCS, from any responsibility resulting in any loss, damage, injury, including death, that may occur to me, or my property, while participating in this event, and/or on the premises. Therefore, no person involved with NCS, in any manner, will be held accountable for any injuries to myself or my property, while participating in this competition, my spouse, or any member of my family, friends, associates, or legal representatives are bound by this release, and is considered as a release, waiver, and covenant, and will not sue any of the above named persons or its affiliates. I also sign, release and acknowledge that all video recordings, use of materials, images, including any and all claims for libel, defamation, right of publicity, recorded auditions/rehearsals/backstage/ competition can be used by NCS to distribute any way that they see fit, whether the sale of video recordings from the show, can be distributed to newspapers, social media sites, radio, magazines, or any means necessary for distribution, and will not be obligated to pay anyone involved with the distribution of said materials. I completely understand that there will not be any monies or any other type of consideration, in any form, will become due to anyone involved in the making of the videos or CDS, or any type of recordings of contestants and anyone involved with the production of NCS, my family, heirs or agents, and any images or likeness or voice can and will be used, with my permission for distribution. Upon signing this release, I understand and comply with all the rules and regulations established by NCS. I have read and completely understand everything that have been discussed in this release, I voluntarily signed this agreement, free of coercion, verbal statements, and fully agree to the terms established in this release. I am aware that because this is a singing competition held publicly and on the internet, there will be photographing, videotaping my image and/or recording my voice. I do not expect payment from anyone associated with NCS for my participation in this competition, and I will refrain from suing anyone affiliated with NCS now or ever. You hereby agree to a11 of the terms established by NCS, and certify that I have read the established ruIes, regulations completely, regarding my participation in the North Carolina Stars Competition relating to auditions, performance, judging, and prizes. I am signing this consent because I agree to allow NCS to release any and all information in relation to my bio, photos, videos…….before and after the audition process, during rehearsals, and my performance during the competition, and can be released to any marketers, advertisement companies or in any type of promotional manner. I do not expect earnings from the distribution, videos taping or voice recordings, or anything related to this competition, and I have agreed to pay all fees required to audition and/or compete. I fully understand that all fees are nonrefundable and does not guarantee a position in the competition, and all fees are required only for the audition and application process. I agree that only the ages of 18-40 are permitted to audition and that all contestants must be residents of the U.S.. I have read and fully understand that the cutoff for contestants will be at 500 and no one will be allowed to apply after this number has been successfully approved. NO family or friends will be allowed inside for the auditions.
